The Hepatitis C Virus (HCV)/HIV Chronic Condiion Coordinator will be responsible for contacting patients who test positive, ensuring patients receive appropriate lab tests and ongoing clinical care as well as linking to patients to appropriate care and resources as approprite and needed. The coordinator will also be responsible for ongoing tracking of all LifeLong Hep-C positive paients. This poisiton while reporting directly to the Community Chief Nursing Officer will work closely with our physician with expertise in liver disease and Hepatitis C diagnosis and treatment.
Contacts patients who test positive, ensuring patients receive appropriate lab tests and ongoing clinical care.
Works with care teams to manage any prior authorization processes for patients requiring treatment.
Responsible for linking patients who have already tested positive for HCV but may not have been linked due to challenges with access.
Manages the tracking of all Hep-C positive patients.
Maintains registry of patients referred for Hep-C treatment, waiting to receive treatment, and on treatment.
Assists with the care for Hep-C patients seen at the Lifelone clinics (this would include following patients who have been prescribed Hep-C treatment but not yet receiving it, as well as those on treatment) both with in-person and through phone contact.
Assists with patient education and follow-up at the various clinics that provide HCV care.
Assists with the development and administration of Primary Care Physician trainings.
Performs data management and analysis.
Disseminates to all clinical staff policies and procedures for HCV testing and linkages to care.
Completes and presents monthly data reports required by the funding agency.
Maintains the study budget.
Bachelor's Degree in related field or 2 years of Hep-C experience with direct patient care.
Minimum Work Experience:
Two years of Hep-C experience iwht dreict patient care.
Previous work experience providing servcies to diverse populations including persons who are disabled, homeless, substance users, nad/or psychologically impaired
Community or Public Health experience.
Required Skills, Knowledge, and Abilities:
Excellent communication skills, verbal, written and interpersonal
Administrative experience in health or social service setitng
Proficient in Microsoft office suite with ability to manage databases.
Previous work experience providing servies to diverse populations including persons who are disabled, homeless, substance users, and/or psychologically impaired.
