Overview

The Remote Patient Monitoring (RPM) coordinator is responsible for development, growth, and promotion of the Remote Patient Monitoring program within the ARH system. The RPM program utilizes innovative workflows and emerging technologies to enhance patient care and wellness within ARH and throughout Southeast Kentucky and Southern West Virginia. The RPM Coordinator, will serve as a clinical resource and develop, implement, evaluate and redesign the RPM program workflows and measures to ensure optimal integration into the delivery of care and service.

Responsibilities

  • Positively contributes and interacts as a team member in the development, growth, and sustainability of the Remote Patient Monitoring Program.
  • Evaluates RPM opportunities to determine feasibility and supports business plan development.
  • Promotes and markets RPM programs both within ARH and beyond.
  • Serves as a subject matter expert and resource for the organization advising staff in RPM aspects of care delivery and operations.
  • Develops evidence-based standards and best practices for RPM.
  • Leads administrative and clinical stakeholders to develop enterprise RPM standards, policies, and procedures.
  • Interviews clinical and administrative staff to analyze clinic workflows allowing for the integration of RPM.
  • Oversees the development of RPM educational materials
  • Monitors activity of RPM programs to identify where improvements may be needed and develops action plans.
  • Provides leadership for consistent delivery and implementation of RPM program.
  • Independently determines priorities based on program guidelines, handles multiple projects of varying scope simultaneously without oversight, and keeps leadership informed of progress.
  • Develops and maintains working relationships with, and acts as the primary contact for, appropriate internal and external staff to ensure high levels of patient and provider satisfaction and efficient operations.
  • Maintains knowledge of departments involved in providing RPM and serves as a liaison with all key stakeholders, regularly soliciting feedback.
  • Develops a working knowledge of local and regional healthcare providers and organizations.
  • Develops a working knowledge of the mission, vision, and values of ARH to represent these characteristics through the RPM program.
  • Local and regional travel may frequently be required.
  • Performs other duties as assigned

Qualifications

Education: Graduate of an accredited school of nursing (RN) preferred

Minimum Work Experience: Three years of professional experience with telehealth or in a healthcare setting.

Required Skills, Knowledge, and Abilities:

  • Ability to maintain confidentiality with all aspects of information in accordance with the practice, State and Federal regulations.
  • Ability to work both independently and collaboratively as a team member to ensure consistent quality of services that support a high level of patient and provider satisfaction.
  • Computer knowledge (Microsoft Word, Microsoft Excel, Practice Plus).
  • Great Communication Skills, Positive Attitude, Self-Motivated, Organizational skills, Medical terminology, Problem solving, Patient Advocate, Capable of multi-tasking, good typing skills, must be a team player.
