The Medical Records Clerk is responsible to assemble, review for completeness, compile and type medical information and data for patients' records and statistical reports.
Maintains surveillance of any incomplete records, assembles and makes quantitative and qualitative analysis for the physician and/or others to complete.
Assures confidentiality of medical records and reports.
Files new registration and admission cards from previous day.
Researches history unit numbers and pulls charts for physicians, admissions, emergency room, etc.
Analyzes daily census report as required.
Answers telephone, screens requests, and answer requests for medical information within prescribed limitations.
Checks list of discharged patients and ensures receipt of all records of patients discharged from hospital.
Furnishes copies of diagnostic reports to appropriate department as requested.
Delivers inpatient records to the floors and picks up charts of discharged patients from hospital.
Assembles and files medical records in accordance with standard medical procedures.
Compiles periodic and statistical reports and other data as directed.
May prepare burial transits and prepare and process birth certificates as required using current word processing program.
Operates various office machines such as photo copiers, postage meters, adding machines, computer terminals and fax machines, etc.
May process department mail and dispatch to appropriate area.
Performs other related duties as assigned.
EDUCATION, TRAINING, AND EXPERIENCE:
High School Diploma or GED equivalent with Business courses (filing, office methods, business English) required. Must type thirty-five (35) words per minute accurately.
Related job experience may be substituted for formal academic preparation when formal training is other than required.
Consistent clerical perception required to spot important detail in material and to avoid errors in computation; errors in general.
Three (3) months on-the-job training will be required to attain proficiency of performance of assigned duties and responsibilities.

Must possess physical ability to endure moderate standing and walking, prolonged sitting, lifting, carrying; exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move object
