The Caseworker is responsible for managing the aspects of patient care that relate to the financial and emotional conditions of the patient and other associated individuals. Establishes and maintains referrals and communications between the patient, family, and community agencies.

Interviews patient and/or relatives to obtain information relevant to medical problems and plans for treatment.   Assists patient system as needed to maximize benefits from medical care by addressing  identified problems or other issues.   Acts as a liaison with clinical staff, patient, relatives, and appropriate outside agencies.   Participates in guiding the patient and family throughout the continuum of care.   Maintains information on community health and welfare resources.   Participates in training sessions with other personnel.   Documents concisely and accurately patient care activities in the medical  record.   Maintains timely statistical reports.   Performs other duties as assigned.

Bachelors Degree in human service area required

 

1 year of healthcare experience preferred
