Overview

The Rotating Pharmacist is responsible for providing quality pharmaceutical care/services to patients and health care professionals at any ARH or affiliated unit as assigned.

Responsibilities

Supervises and performs the functions involved in the inpatient drug distribution program.

 

        Supervises and performs the functions involved in the accurate and efficient dispensing of outpatient prescriptions.

 

        Supervises and performs the functions involved in the I.V. Admixture Program to include the accurate and efficient preparation and delivery of total parenteral nutrition solutions, chemotherapy, antibiotics, and other medications for intravenous/irrigation therapy.

 

        Reviews and verifies medication orders for accuracy and completeness.

 

        Monitors patient's drug therapy to detect/prevent drug interactions, contraindications, and other adverse reactions.

 

        Makes appropriate interventions to improve patient care.

 

        Uses and is proficient in the operation of the pharmacy computer system.

 

        Compounds and dispenses medications and other pharmaceutical agents/products.


        Obtains pertinent patient medication/medical information.

 

        Provides current and accurate drug information to patients and health care professionals.

 

        Maintains appropriate records and inventory on all drugs/pharmaceutical agents.

 

        Monitors the proper storage and distribution of patient floor stock, controlled drugs, and emergency medications.

 

        Communicates effectively with patients and health care professionals.

 

        Trains and supervises pharmacy supportive personnel as assigned.

 

        Serves on hospital committees as assigned.

 

        Assumes the duties of the Director of Pharmacy when assigned.

 

        Provides pharmaceutical care in assigned locations as directed by Central Pharmaceutical Services.

 

                 Performs other related duties as assigned.

Qualifications

EDUCATION, TRAINING AND EXPERIENCE:

 

        Bachelor of Science degree in Pharmacy, Pharm D., or other advanced degree.

 

        Licensure eligibility in states where expected to practice pharmacy.

 

POSITION PHYSICAL REQUIREMENTS:

 

        Must possess the physical ability needed to exert 20 to 50 pounds of force occasionally; and/or 10 to 25 pounds of force frequently; and/or greater than negligible up to 10 pounds of force constantly to move objects.  Physical Demand requirements are in excess 

        of those for light work.

 

SPECIFIC ACCOUNTABILITIES:

 

        At any ARH or affiliated unit:

 

        Responsible for properly assessing patient's pharmaceutical needs and providing pharmaceutical care.

 

        Responsible for the accurate and proper compounding and dispensing of prescribed medications and solutions.

 

                 Responsible for the proper inventory and record keeping of medications

 

        Responsible for properly counseling patients/patients' families, and appropriate health care professionals regarding drug therapy.
