The physical therapist is accountable for conducting and supervising physical therapy programs to enable the
patient to reach the maximum physical performance possible, prevent disability and to teach the patient and
family to function within the scope of any permanent physical disability.
Demonstrates clinical knowledge and skills necessary to provide physical therapy services appropriately to the
age of the patient served.
Completes a comprehensive evaluation of physical function of assigned patients based upon clinical diagnosis,
impairment and/or injury in accordance with organizational and state practice act requirements.
Formulates physical therapy treatment plan based upon physical assessment/evaluation.
Administers skilled physical therapy treatment in accordance with organizational and state practice act
requirements.
Coordinates patient care with other clinical team members, community resources and family members as
indicated, ensuring optimal patient care and communication.
Completes all required paperwork following organizational and state practice act requirements to ensure that
documentation reflects care provided and the outcomes achieved.
Maintains and enhances personal knowledge as well as promoting professional standards.
Provides supervision to physical therapist assistants and/or physical therapy workers to promote continuity of
care.
Demonstrates behaviors specific to job that provides quality service in a safe and secure work environment.
Assumes the duties of managing the department in the absence of the director.
Bachelor's Degree Graduate of an accredited school of physical therapy (minimum BS degree). Foreign-trained therapists must have their credentials reviewed and approved by an independent agency appointed by the State Board of Physical Therapy. Required
1-3 years New graduates considered, 2 years' experience preferred.
Licensed, or eligible for licensure, as a physical therapist by state where employed Upon Hire Required
