Overview
- Assess patients for discharge needs, utilizing pre-established criteria, and plan appropriate discharge planning for the patient, involving the patient and family in the plan of care.
- Make patient rounds with the physician and nursing staff, if necessary.
- Chart audits and reviews.
- Prepare reports as outlined in the job description.
- Attend Utilization Review meetings.
- Attend daily Length of Stay meeting.
- Follow criteria as outlined by the Kentucky Medical Review Organization, which mean reviewing 100% of all Medicare charts daily for severity of illness and intensity of service.
- Comply with HIPAA regulations.
- Participate in the Hospital’s Quality Service Management Plan.
- Assist Physician in maintaining records, if necessary.
- All other duties as outlined in the job descriptions or as assigned.

Qualifications
- Must be a Registered nurse, with an active Kentucky License in good standing, with no restrictions.
- Must have at least 3 years nursing experience.
- Good communication skills, both written and verbal.
- Ability to work well with physicians and ancillary departments.
- Capable of functioning independently.
- Must have computer skills.
