The Registered Nurse requires substantial specialized knowledge, judgement and nursing skills based upon the principles of psychological, biological, physical, and social sciences in the application of the nursing process in the respective departments in which assigned. The RN must collaborate with physicians and multidisciplinary team members; provide physical and psychological support to patients, friends, and families; supervising assigned team members and students.
• Associate's Degree required.
• Bachelor's Degree preferred.
• Registered Nurse required.
• BLS certification required upon hire.
• De-Escalation Training required within six (6) months of hire.
• Driver's License required.
• Responsible for preparation of and interpretation of client plans of care and systematic assessments/reassessments.
• Responsible for performing nursing treatments, procedures, and total client care in accordance with established agency and nursing care standards.
• Responsible for communicating pertinent client assessment/reassessment findings to physicians.
• Responsible for initiating referrals to other healthcare organizations and community services based on identified client needs and physician orders.
• Ability to travel to client residences in inclement weather and provide nursing care in varying environmental situations.
• Strong clinical assessment, care planning, and critical thinking skills.
• Ability to work independently while coordinating care with physicians, patients, families, and interdisciplinary healthcare teams.
• Effective verbal and written communication skills.
• Ability to prioritize patient care needs and manage multiple responsibilities in a home health environment.
• Ability to maintain accurate and timely clinical documentation.
• Ability to establish effective working relationships with patients, families, physicians, and community resources.
