Serves as a liaison between ARH, physicians, patients, and hospital staff to promote awareness of ARH services and programs. Educates professional and non-professional hospital personnel on available ARH resources, facilitates communication with physicians in the inpatient setting, and supports coordination of care for hospitalized ARH patients. Responsible for developing, implementing, monitoring, and managing strategies that drive referrals across the ARH network, while assisting with referrals to all ARH service lines. Provides case management oversight for hospitalized ARH patients and collaborates with interdisciplinary teams to ensure continuity of care and optimal patient outcomes. Reports directly to the AVP Marketing Manager.
• Maintains effective working relationships with healthcare referral sources.
• Accepts requests for services and verifies appropriateness for ARH services.
• Identifies potential learning needs of referral sources and develops resources to help meet those needs.
• Assists with the development and implementation of methods for tracking referral trends.
• Develops and maintains relationships with designated referral sources within the hospital setting.
• Provides oversight and case management support for ARH patients in an inpatient setting.
• Educates the medical community, including physicians, nursing homes, nursing agencies, and other referral sources, regarding ARH services and programs.
• Meets regularly with hospital discharge planners and social workers to provide information about ARH services and facilitate care coordination.
• Acts as a liaison and advocate for ARH patients admitted to hospitals.
• Develops and maintains an ARH presence within area hospitals by attending rounds and medical staff meetings when appropriate, conferring with providers, participating in in-service programs, and assisting with new staff orientation.
• Demonstrates cooperation in adhering to administrative policies and supports the mission, vision, and values of ARH.
• Shares information, knowledge, concerns, and expertise through staff meetings, in-service education, and the orientation and education of new employees.
• Establishes and maintains effective communication using tact, sensitivity, sound judgment, and professionalism when interacting with patients, families, coworkers, supervisors, healthcare professionals, and physicians.
• Performs additional duties as assigned by the AVP Marketing Manager.
• Graduate of an accredited school of professional registered nursing.
• At least two (2) years of general nursing experience is required.
• Current home health experience and/or medical-surgical experience preferred.
• Current critical care experience preferred.
• Currently licensed as a Registered Nurse through the State Board of Nursing.
• Knowledge of professional nursing principles and practices.
• Ability to assess, plan, implement, and evaluate patient care.
• Ability to work effectively with patients, families, physicians, and interdisciplinary healthcare teams.
• Strong communication, organizational, and documentation skills.
• Ability to function independently and exercise sound clinical judgment in a variety of patient care settings.
