The Site Coordinator (SC) will support an innovative care delivery model, Rural Home Hospital.  The program offers a subset of patients an alternative to standard hospital admission, in which patients can receive highly specialized acute-care services in their home. The role also has follow-up functions of both hospitalized and post-discharge patients via telephone.  For example, the SC calls patients to collect data for post-discharge surveys.
