The Licensed Diabetes Educator (LDE) is responsible for delivering high-quality, evidenced-based diabetes self-management education and support (DSME) to individuals and groups across the service area. This role combines direct patient education, community outreach, and program support, with a strong emphasis on improving access to diabetes care, reducing health disparities, and supporting patients in both clinical and community-based settings. The LDE will work collaboratively with interdisciplinary healthcare teams and community partners while representing the diabetes program at outreach events and educational activities.

Provide leadership for the Emergency Departments, Cath Lab, Telemetry, Observation Unit and Critical Care Unit along with other entities within the cardiac service line to promote excellence in cardiac patient care. This position will also assist in the planning, development and implementation of program goals and operations. Coordinates and oversees the ongoing Chest Pain Accreditation to improve care of the cardiac patient population.
