The Care Navigator will be responsible for contacting, through telephone calls, patients that meet the criteria for the Chronic Care Management program at ARH.The coordinator will manage the intake process for new patients, coordinating referrals, scheduling appointments, and gathering necessary information to facilitate healthcare services, acting as the initial point of contact for patients and providers. The Chronic Care Management program is designed to promote and encourage healthy lifestyles to patients that have multiple chronic diseases.

The Kitchen & Cafeteria Worker is responsible for performing duties relevant to operating the dishwashing machine, transporting dishes, food item preparation, patient meal service, cafeteria customer service, and cleaning of the preparation and service area.

The Peer Recovery Coach provides non-clinical services intended to aid patients in establishing recovery from high risk of dependent alcohol and drug use problems. The Peer Recovery Coach will work as part of the Emergency Department team/or designated area to provide peer support and motivation to encourage patients who are seeking treatment for alcohol or drug dependency and addiction issues. The Peer Recovery Coach will assist in completing referrals to treatment services and developing service plans to promote successful linkage to treatment services. Services will be provided in the Emergency Department/or designated area with appropriate follow-up.
